Immunizations

As part of the initial registration process, scholars who are MDs and/or PhDs and have patient contact are also required to complete an Immunization Record form (PDF format). It is the scholar's responsibility to forward the completed immunization form to the Insurance Coordinator and keep annual tests up-to-date while appointed as a Stanford University postdoctoral scholar with patient care responsibilities.
