Help Using Forms

Except where indicated, the forms linked from the Forms page are fillable PDF (Adobe portable document format) forms. There are two ways you can use these forms:

1) Using the free Adobe Acrobat Reader. The free reader is available for download from the Adobe site. Some things to keep in mind when using Reader:

Reader, which can work as a plug-in for your browser or as an independent application, allows you to print out the un-filled form. In this case you would have to type or write the information into the form by hand.

With Reader you can also fill in the active fields of the form with your text, and then print it out. However, when you close the document, your changes are not saved.
