Change of Status of a Postdoctoral Scholar

The Office of Postdoctoral Affairs needs to be notified of any change in status of a postdoctoral scholar as soon as possible. There are two separate forms now to report these changes to us.

Termination forms PDF icon should be provided to the Office of Postdoctoral Affairs a month before the scholar is leaving. A forwarding address is required. The form will end registration and end benefits coverage. The department is responsible for ending all pay lines associated with the scholar. The scholar will be mailed COBRA (insurance) materials; s/he is eligible to extend existing medical and/or dental insurance coverage. If the termination notice is late, the department may incur charges for premiums beyond the termination date on the form.

Department Transfer form PDF icon In the event a postdoc is transferring from one department to another, both departments need to complete the Change form. The new department must also include the Offer Letter given to the scholar. If the transfer is requested when the scholar has been at Stanford for four years and/or has five or more years of research experience, then FIRST a request must be made for an extension of the appointment. This is done with a letter from the faculty sponsor to the Assistant Dean of Postdoctoral Affairs. The letter must state the reason for the extension, the value of the research training, and the period of time requested.

The forms can be printed and hand-completed. If you have the full version Adobe Acrobat , you can fill in the PDF form, save and as an attachment via e-mail. (This software can be purchased through the CWA.) With the free Adobe Acrobat Reader software, you can fill it in and print and fax only.
