Any terminations due to
inadequate academic progress must follow a process by which progress
discussions and written evaluation by the faculty PI are made. This
process must note the specific steps to be taken in order to remedy any
deficiencies, and the possibility of early termination if those deficiencies
remain by a specific date. Such process takes place several weeks/months in
advance of taking this early termination action.

Dear Dr. [Scholar Name]
This is to notify you that
your postdoc appointment will end earlier than the expected contractual end
date of [appointment end date] due to [state reason here: a
loss in funds from the “grant/contract name” OR insufficient progress as
documented by our conversation and my letters of XXXXXX.] This
means that I must make a difficult decision and end your postdoc appointment
effective [early termination date].
Your Postdoc appointment
and health insurance benefits will end on [early termination date].
After that date, you will have the option to continue your health coverage
through COBRA for up to 18 months. CONEXIS, postdoc COBRA Administrator,
will mail you a COBRA packet to the address you have listed in the Office of
Postdoctoral Affairs. The packet will contain instructions on how to
enroll for COBRA and will also include the COBRA rates (full premium cost plus
a 2% administrative fee). You will have 60 days from your termination
date to enroll for health benefits through COBRA, and the COBRA coverage begin
date will be retroactive to the date you lost coverage. For additional

Please be aware that in
the event [early termination date] date occurs and you have not
identified a new postdoc appointment, funding has not been restored or you have
not found another position, you may request a leave of absence for up to ninety
(90) days through the Office of Postdoctoral Affairs. If an unpaid leave
is approved, and because I am initiating your appointment end date prematurely,
the department will continue to assume the full cost of your health care
benefits during any approved leave period. In that case, the termination
date of your Stanford affiliation will be the end date of your approved unpaid
leave and your COBRA eligibility (described above) would commence at that time.
